Intravascular Ultrasound (IVUS)

Intravascular Ultrasound is a technique that employs real-time ultrasound within a blood vessel. A catheter is fitted with an ultrasound apparatus that picks up images via sound waves as it travels through the vascular system. IVUS provides a 360-degree panoramic view inside the blood vessel, revealing plaque formation on the arterial wall. This enables Dr. Broadway to comprehend the changes that occur in your vessels prior to and following treatment. Minimally invasive surgery is a modern technique that allows surgeons to limit the size and number of incisions or cuts needed to perform surgery. Compared to traditional open surgery, it is generally considered safer as it results in a faster recovery and less discomfort. In open surgery, the surgeon makes one large incision to access the area they need to operate on. However, minimally invasive surgery is performed through a small needle without any notable incisions.

All Vascular procedures at Broadway Vascular are performed with this minimally invasive technique. Broadway Vascular specializes in treating narrow or obstructed blood vessels with various methods. Atherectomy, also known as the “roto-rooter device,” is a technique that involves removing plaque from an artery, creating more space for blood to flow. Another technique is angioplasty, which involves inflating a balloon inside the artery to open it up. Stenting is a procedure only used as a last resort to reestablish proper blood flow. It involves inserting a short wire mesh tube into the artery during the procedure, which is left permanently to allow blood to flow more freely.

Renal Artery Ablation

Renal denervation, known as renal ablation, is a minimally invasive procedure performed at Broadway Vascular. This procedure is used primarily to treat hypertension that has not responded to other treatments. Patients who undergo this procedure usually experience lower blood pressure. They further reduce the risk of heart failure, kidney disease, and stroke. In many cases, patients can significantly reduce their hypertension medication dosage or even stop taking medication altogether after undergoing this treatment.

Your brain, heart, and kidneys send messages throughout your body to regulate your blood pressure. However, when these nerves become overactive, they can increase your blood pressure. Renal denervation helps to reduce the activity of the renal nerves in the kidneys, which lowers blood pressure. During the procedure, a catheter used sends either ultrasound or radiofrequency (heat) energy to the renal arteries, the blood vessels that supply blood to the kidneys. This energy destroys (ablates) the renal nerves without damaging the arteries. As a result, nerve activity reduced, leading to a drop in blood pressure.

Spinal Cord Stimulation

Neuromodulation therapy uses targeted electrical stimulation or medication to the central, peripheral, or autonomic nervous system to relieve pain. Spinal cord stimulation has been a treatment option for pain for over 50 years. Painful diabetic neuropathy affects approximately 15-25% of people with diabetes, and conventional medical management, including medications, often does not work for many patients. Embolization

Embolization is a minimally invasive procedure involving small particles, such as gelatin sponges or beads, to block a blood vessel. The purpose of embolization is to stop bleeding or prevent blood flow to an abnormal tissue or tumor. This method is commonly used to treat various health conditions such as liver cancer, kidney cancer, and neuroendocrine tumors. At Broadway Vascular, embolization is used to treat a variety of ailments such as uterine fibroids, benign prostatic hyperplasia, knee pain, cancer, and bleeding bowel, among others.

Vertebroplasty & Kyphoplasty

Vertebroplasty and kyphoplasty are minimally invasive procedures used to treat painful vertebral compression fractures (VCF) in the spinal column. These fractures occur when the rectangular shape of a vertebral body is compressed, causing pain. VCFs usually occur in individuals with osteoporosis, a disease that weakens bones and makes them more vulnerable to breaking. VCFs can also occur due to cancer weakening the vertebrae.
During vertebroplasty, a cement mixture is injected into the fractured bone through a hollow needle, using image guidance such as fluoroscopy. For kyphoplasty, a balloon is first inserted through the hollow needle to create a cavity in the fractured bone, which is then filled with cement after the balloon is removed.
These procedures are typically used to treat painful VCFs caused by osteoporosis, cancer that weakens the vertebrae, or in patients who may have impaired bone healing after a fracture, osteoporosis due to long-term steroid treatment, or a metabolic disorder.
Pain relief may be immediate or occur within two days following the procedure. Pain resulting from the procedure should diminish within two to three days. Vertebroplasty and kyphoplasty can increase a patient’s functional abilities and allow them to return to their previous activity level without physical therapy or rehabilitation. Port-A-Cath

Long-term intravenous infusion treatments are required for many disease processes, whether daily, weekly, monthly, or longer. However, needle IV access can be painful and destructive to veins, making it very difficult to gain IV access, requiring many needle sticks. The most common and well-known long-term therapy and disease process is chemotherapy for cancer. But other chronic diseases, such as rheumatoid arthritis and sickle cell disease, also need long-term intravenous therapy.
To make the process less painful and harmful to the veins, a port-a-cath (Port) is used. It is a small device placed under the skin and attached to a vein. When IV access is needed for therapy, the Port is accessed with the needle instead of the vein, causing minimal pain to the patient and no damage to the vein. The port-a-cath can remain in place as long as the treatment is needed, and when it is no longer needed, it can be removed quickly and easily.
The Port placement procedure is generally painless and takes only 30 minutes to complete. It is done in the medical office with local and sometimes moderate sedation. The patient can go home usually 30 minutes after the procedure is finished. There is very little post-procedural pain or complications.

Ablation Therapies

Ablations are advanced, minimally invasive procedures that effectively treat benign and cancerous tumors in organs such as the liver, kidney, and thyroid glands. The procedure utilizes three types of ablation techniques, including radiofrequency ablation (RFA), microwave ablation (MA), and cryoablation (CA), which use heat and cold to destroy the tumor cells.
